About The Position

The Clinical Care Leader (CCL) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the home setting. This role collaborates with other inter-professional colleagues to plan, implement, and evaluate care. The CCL documents resident history, symptoms, medications, and care provided in compliance with applicable standards and regulations. They utilize technology to enhance resident care, collaborate with team members, delegate to and supervise unlicensed personnel, assume overall nursing accountability for a resident assignment, and serve in leadership roles on department/organizational task forces and committees. The CCL demonstrates competency and practices within the full scope of nursing expertise and knowledge, utilizing appropriate age and population-specific standards. Effective communication skills are essential for collaboration, negotiation, and conflict resolution with peers, colleagues, residents, and families. Strong clinical reasoning and critical thinking skills are required to analyze data and respond to changing situations, including care coordination, resident assessment, resident education, triage, and various other nursing interventions. The role functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in the state of practice and licensure, recognizing the American Nurses Association's Scope and Standards of Practice and Code of Ethics for Nurses.

Requirements

  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Currently holds an unencunmbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific competencies and certifications.
  • Basic Life Support (BLS) required.

Nice To Haves

  • Bachelor's degree in Nursing strongly preferred.
  • Minimum of two years’ clinical experience preferred.
  • Depending on location, a valid drivers license may be required.
  • Resident Assessment Instrument (RAI) process experience preferred, but not required.

  • Utilizing appropriate age and population specific standards as designated in the specific care area or unit.
  • Communicating effectively to collaborate, negotiate and resolve conflicts, situations, and/or issues with peers, inter-professional colleagues, residents and families.
  • Utilizing strong clinical reasoning and critical thinking skills to analyze data and appropriately respond to changing situations.
  • Performing care coordination, resident assessment, resident education, triage, and various other nursing interventions.
  • Functioning within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure.
