About The Position

The Clinical Care Leader (CCL) at Sanford Health is responsible for providing individualized nursing care in the home setting, utilizing the full nursing process including assessment, diagnosis, outcomes/planning, implementation, and evaluation. This role involves collaboration with inter-professional colleagues to plan, implement, and evaluate care, as well as documenting resident history, symptoms, medications, and care in compliance with standards. The CCL uses technology to enhance resident care quality, effectively delegates to and supervises unlicensed personnel, and assumes overall nursing accountability for assigned residents. They also serve in leadership capacities on department/organizational task forces and committees. The position requires demonstrating competency within the full scope of nursing expertise, applying age and population-specific standards, and possessing strong communication, clinical reasoning, and critical thinking skills for collaboration, conflict resolution, data analysis, and responding to changing situations. The CCL must function within the scopes and standards of nursing practice as outlined by the Nurse Practice Act and Administrative Rules in the state of practice and licensure, adhering to the American Nurses Association's Scope and Standards of Practice and Code of Ethics for Nurses.

Requirements

  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state
  • Obtains and subsequently maintains required department specific competencies and certifications
  • Basic Life Support (BLS) required

Nice To Haves

  • Bachelor's degree in Nursing strongly preferred
  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA)
  • Minimum of two years’ clinical experience preferred
  • Depending on location, a valid drivers license may be required
  • Resident Assessment Instrument (RAI) process experience preferred, but not required

  • Utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care in the home setting
  • Collaborating with other inter-professional colleagues, to plan, implement and evaluate care
  • Documenting resident history, symptoms, medications and care provided in compliance with applicable standards/regulations
  • Utilizing technology to enhance quality resident care
  • Collaborating with inter-professional team members
  • Effectively delegating to and supervising unlicensed personnel
  • Assuming overall nursing accountability for a resident assignment
  • Serving in leadership roles on department/organizational tasks forces and committees
  • Demonstrating competency and practicing within the full scope of nursing expertise/knowledge and utilizing appropriate age and population specific standards as designated in the specific care area or unit
