About The Position

Tri-Cities HH Care, an Amedisys partner, is hiring a full-time registered nurse case manager for home health. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with an on-call weekend rotation. Mileage is reimbursed. The role focuses on helping patients heal at home through assessment, care planning, patient education, coordination with the care team, and supervision of LPNs and home health aides. Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines based on patient needs, or document the rationale for not making referrals.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
