This position is for an RN Case Manager within the Scotland Memorial Hospital - Hospice department. The role involves providing nursing services, including ongoing assessments of patients and their needs, in accordance with the Nurse Practice Act, Hospice policies and procedures. Key aspects include making regular home visits, assisting with 24-hour on-call services, performing prescribed treatments, conducting physician consults, and developing/implementing nursing plans of care with an emphasis on symptom management, comfort care, and patient/family education. The RN Case Manager will work collaboratively with the Director of Clinical Operations and other interdisciplinary team members to develop individualized Plans of Care and ensure timely and accurate documentation. Professional development through workshops and mandatory education is also expected.
