About The Position

This position is for an RN Case Manager within the Scotland Memorial Hospital - Hospice department. The role involves providing nursing services, including ongoing assessments of patients and their needs, in accordance with the Nurse Practice Act, Hospice policies and procedures. Key aspects include making regular home visits, assisting with 24-hour on-call services, performing prescribed treatments, conducting physician consults, and developing/implementing nursing plans of care with an emphasis on symptom management, comfort care, and patient/family education. The RN Case Manager will work collaboratively with the Director of Clinical Operations and other interdisciplinary team members to develop individualized Plans of Care and ensure timely and accurate documentation. Professional development through workshops and mandatory education is also expected. Requirements

  • Licensed as a Registered Nurse
  • 1 Or More Years In Hospice Or Home Health

Responsibilities

  • Provides nursing services, under the direction of the Director of Clinical Operations including on-going assessments of patients and their needs, in accordance with the Nurse Practice Act, Hospice policies and procedures and other applicable standards.
  • Making regular home visits on assigned patients.
  • Assisting in providing 24 hour on-call services, as scheduled.
  • Performing prescribed treatments and procedures, in compliance with the established guidelines of the NC Nurse Practice Act; as per the South Carolina Nurse Practice Act, Registered Nurses may give medications.
  • Conducting physician consults, as needed, to obtain orders, provide patient updates, and review the plan of care.
  • Assisting in performing initial and on-going nursing assessments of patient/family needs.
  • Developing and implementing the nursing plan of care, with appropriate updates providing hospice nursing services with emphasis on: identifying the patient/family’s physical, psychosocial and spiritual needs addressing symptom management and comfort care needs providing teaching and education to the patient/family documenting problems, appropriate goals, interventions and the patient/ family response to care, and assisting with the coordination of services.
  • Works with the Director of Clinical Operations and other interdisciplinary team members in developing an individualized Plan of Care for assigned patients.
  • Interacts as a member of the IDT by: Communicating with other team members; Timely reporting of status changes in patients; Regular attendance and participation in staff and team meetings.
  • Assists in keeping patient/family records updated and provides timely/accurate documentation of all services rendered.
  • Assumes responsibility for professional development by attending workshops, in-service education programs and mandatory education events, whenever possible.
