The RN Case Manager is responsible for initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, psychosocial, and environmental needs and ADLs. This includes assessing for risks such as grief, and considering cultural and spiritual needs, as well as verbal and non-verbal communication. The role involves implementing the individualized Plan of Care (POC) and recommending revisions as necessary. The RN Case Manager collaborates with the patient/caregiver, physicians, and other healthcare professionals to coordinate care. They attend Inter-Disciplinary Group (IDG) meetings, participate in the patient care planning process, and prioritize needs to promote coordinated patient care. The position also includes performing on-call responsibilities and providing on-call service to patients/families as assigned. Timely submission of all documentation is required, along with ensuring overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures. Other duties as required will also be performed.
