About The Position

The RN Case Manager evaluates patients’ medical, psychosocial, and discharge needs upon admission and throughout their hospital stay. The role develops and implements individualized care plans with the healthcare team, coordinates appropriate discharge planning, and monitors hospital resource use to support appropriate care. The RN Case Manager also educates patients and families about care plans, post-discharge instructions, and available resources, and advocates for care aligned with patients’ needs, preferences, goals, and values. This is a part-time position at Harlingen Medical Center in Harlingen, Texas, with day shifts consisting of 8-hour shifts.

Requirements

BSW or another bachelor’s degree in a related field.
At least one year of experience in case management, discharge planning, or nursing management.
RN license.
Experience and knowledge in basic to intermediate computer skills.

Nice To Haves

Minimum of five years of acute care experience preferred.
CCM certification, or ability to obtain it within one year.
Current BCLS certificate preferred.
Knowledge of Milliman Criteria and InterQual Criteria preferred.

Coordinate safe and appropriate discharge plans, including referrals to rehabilitation, home health, or long-term care facilities.
Monitor the use of hospital resources and services to ensure appropriate care and avoid unnecessary treatments or extended stays.
Help patients and families understand care plans, post-discharge instructions, and available resources.
Advocate for patient needs and preferences, ensuring care is aligned with their goals and values.
