About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Registered Nurse (RN) Case Manager in Clewiston, Florida. This is a community-based RN Case Manager role focused on patient advocacy, care planning, and coordination of complex medical needs. The role offers the opportunity to conduct assessments within home and clinic settings while supporting long-term patient outcomes in an outpatient nursing environment centered around care coordination and interdisciplinary collaboration. Requirements

• Active Florida RN license REQUIRED
• Comfortable conducting assessments within home and clinic settings REQUIRED
• Strong care coordination and patient advocacy skills REQUIRED
• Team-based care approach REQUIRED
• Comfortable managing diverse patient populations REQUIRED
