About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Registered Nurse (RN) Case Manager in Beverly, Massachusetts. This is a hybrid role supporting aging adults through community-based care coordination and individualized care planning. The position offers the opportunity to work independently without bedside nursing demands, focusing on continuity of care, service coordination, and long-term patient support within a home-based setting. The hybrid structure combines community visits with remote care coordination responsibilities within a mission-driven senior care environment with strong interdisciplinary collaboration.

Requirements

• Active Massachusetts RN license REQUIRED
• Minimum 2 years RN experience REQUIRED
• Strong communication and organizational skills REQUIRED
• Comfortable working independently in community settings REQUIRED
• Team-based care approach REQUIRED
• Comfortable with hybrid remote/community workflow REQUIRED

• Home care experience preferred

Responsibilities

• Develop individualized patient care plans
• Coordinate community-based services and continuity of care
