About The Position

Boost Home Healthcare is hiring an experienced Registered Nurse Case Manager in Greeley and nearby areas in Colorado. This role allows you to manage your own client caseload, guide a team of caregivers, and build lasting relationships with clients and families, offering more freedom than hospital settings with less politics. Requirements

Active, unencumbered Registered Nurse (RN) license in Colorado
At least 1 year of home care or home health nursing experience (required)
Graduate of an accredited school of nursing
Current BLS/CPR certification
Oasis Documentation
Valid driver’s license and reliable transportation
Strong clinical judgment, organization and communication skills, plus a genuine passion for client-centered care

Nice To Haves

Case management, care coordination or caregiver supervision experience (preferred)

Responsibilities

Complete initial and ongoing in-home nursing assessments of each client’s physical, cognitive, psychosocial and home safety needs
Create and update individualized plans of care and make sure caregivers understand and follow them
Oversee a caseload of home care clients, tracking changes in condition and adjusting services as needs change
Serve as the main clinical contact for families, physicians, hospitals and community partners; make referrals when needed
Keep accurate, timely client records and follow Colorado home care regulations, infection control and HIPAA guidelines
Help resolve client and caregiver concerns quickly and professionally
