About The Position
This is a part-time outpatient Registered Nurse Case Manager position in the Houston area. The RN Case Manager works independently as an integral member of the interdisciplinary care team and is responsible for admitting patients and coordinating appropriate care for each patient and family. Main duties include admissions, assessments, discharges, and pronouncements.
Requirements
- State of Texas RN license.
- Effective written and verbal communication skills.
- Ability to travel and reliable transportation.
- Clean background, drug screen, and driving record.
Nice To Haves
- At least 1 year of experience as an RN.
- Experience with hospice or home health.
Responsibilities
- Admit patients.
