About The Position
This is a part-time, out-patient Registered Nurse Case Manager position in the Canton area. The role requires an independent and experienced RN and is an integral part of the interdisciplinary care team. The RN Case Manager is responsible for admitting patients and coordinating appropriate care for each patient and family. Main duties include admissions, assessments, discharges, and pronouncements.
Requirements
- State of Michigan RN license.
- Effective written and verbal communication skills.
- Ability to travel and reliable transportation.
- Clean background, drug screen, and driving record.
Nice To Haves
- At least 1 year of experience as an RN.
- Experience with hospice or home health.
Responsibilities
- Admit patients.
