RN Case Manager
About The Position
Our Case Management team is expanding! We are seeking RN Case Managers to join our team. Those with a background in Case Management or looking to explore a new opportunity in nursing are encouraged to apply! As an RN Case Manager, you will use your clinical expertise to assess patients across the continuum of care, coordinate individualized care plans, and collaborate closely with physicians and multidisciplinary teams. You will advocate for patients, review diagnostic results and level-of-care-needs, and help ensure patients receive the right care at the right time. This is a great opportunity for an RN who enjoys clinical decision-making, care coordination, and making a meaningful impact on patient outcomes.
Requirements
- Minimum of two (2) years of nursing experience.
- Graduate of an accredited nursing program.
- Licensed/registered RN in NY State.
- Maintain current professional licensure in nursing or professional filed of certification.
Nice To Haves
- Bachelor Nursing degree is preferred.
- Experience in a hospital Case Management/discharge planning
- Appropriate certification in the case management preferred (e.g. Commission for Case Management Certification (CCMC) or Association of Rehabilitation Nurses).
Responsibilities
- Coordinates discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment.
- Coordinate the hospital activities concerned with case management and discharge planning.
- Ensure compliance with quality patient care and regulatory compliance.
