About The Position

Four Seasons is a leading provider of hospice and palliative care, recognized for its excellent patient care and commitment to employees. They are seeking an RN Case Manager for the South Region to coordinate the nursing plan of care, provide direct nursing care, and supervise LPN and Hospice Aide staff. The role involves managing a caseload of approximately 15 patients with varying acuity and needs, with support provided for larger caseloads or assistance offered for smaller ones. The position is exempt (salaried) and full-time, working 40 hours per week, Monday through Friday from 8 AM to 4:30 PM, with rotating emergency weekend/holiday call. The primary service area includes Henderson County and surrounding areas.

Requirements

Valid NC RN license
Valid Driver’s License and proof of automobile insurance
Two years of clinical experience preferred
Must be willing and able to care for patients with infectious disease processes unless approved accommodations.
Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation.
Knowledge of HIPAA privacy rules and regulations.
Knowledge of electronic medical records systems.

Responsibilities

Coordinates the nursing plan of care.
Provides direct nursing care.
Instructs and supervises LPN and Hospice Aide staff in the delivery of nursing care to clients.
Manages the nursing care for an assigned caseload of approximately 15 patients with varying acuity and needs.
Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation.
Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens.
Record and report the results of the nursing assessment.
Plan, initiate, deliver, and evaluate appropriate nursing acts.
Teach, assign, or supervise other personnel in implementing the treatment regimen.
Collaborate with other health care providers in determine the appropriate patient and family-centered plan.
Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen.
Provide teaching and counseling about the patient's health.
Report and record the plan for care, nursing care given, and the patient's response to that care.
Supervise, teach, and evaluate those who perform nursing functions and administer nursing programs and nursing services.
Provide for the maintenance of safe and effective nursing care, whether rendered directly or indirectly.
Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies.
Participate in department and staff meeting as required.
Participate in ongoing education and training offered by the agency and/or required by nursing supervisor as part of individual education plan of RN.
Comply with company policies, procedures, guidelines, and Mission Statement.
Practice safety and infection control methods.
Assist in providing 24 hour on-call services, including coverage of at least one holiday per year.
Assess for the need of other disciplines.
Perform other but similar duties as assigned.
