About The Position

We are seeking a dedicated and compassionate PRN RN Case Manager to join our team. This role is crucial in coordinating patient care, ensuring smooth transitions between care settings, and advocating for patient needs. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a passion for patient advocacy.

Requirements

Current RN license in the state of practice.
Minimum of 2 years of experience as a Registered Nurse.
Experience in case management, discharge planning, or utilization review preferred.
Strong understanding of healthcare regulations and reimbursement systems.
Excellent assessment, planning, and organizational skills.
Effective communication and interpersonal skills.
Ability to work independently and as part of a team.
Proficiency in electronic health record (EHR) systems.
Critical thinking and problem-solving abilities.

Nice To Haves

Case Management Certification (e.g., CCM, ACM).
Experience in a specific specialty area (e.g., oncology, cardiology, pediatrics).
Bilingual abilities.

Responsibilities

Assess patient needs and develop comprehensive care plans.
Coordinate patient care services with physicians, nurses, and other healthcare professionals.
Facilitate patient transitions between different levels of care (e.g., hospital to home, skilled nursing facility).
Educate patients and families about their conditions, treatment options, and available resources.
Advocate for patients to ensure they receive appropriate care and services.
Monitor patient progress and adjust care plans as needed.
Maintain accurate and timely documentation of all case management activities.
Collaborate with insurance companies and other payers to obtain authorizations for services.
Identify and address barriers to care, such as financial, social, or logistical challenges.
