About The Position

Vitality Living of New York Mills is seeking a dedicated RN Case Manager to join our team! The RN Case Manager works collaboratively with the interdisciplinary team to ensure quality care is delivered to our residents based on their individual needs. This role provides clinical leadership, management, education, training, and support to the clinical team while helping ensure compliance with current Assisted Living Licensure requirements.

Requirements

• Current, unencumbered Minnesota RN license required.

• Tactful and professional communication skills.
• Dependable, organized, and able to work independently.
• Proficient with technology, electronic medical records, and Microsoft programs.
• Strong ability to multitask and prioritize resident and staff needs.

Nice To Haves

• 3+ years of experience in assisted living or a senior care setting preferred.

Responsibilities

• Conduct initial, routine, and change-in-status assessments as required.
• Develop, maintain, and monitor resident care plans and services.
• Ensure services are appropriately documented and submitted for reimbursement.
• Provide leadership, education, training, and competency support to the clinical team.
• Support quality care and regulatory compliance throughout the community.
• Work collaboratively with residents, families, staff, and the interdisciplinary team.
