About The Position
Independence Home Health LLC is seeking a dedicated and compassionate full-time RN Case Manager to provide quality care and support patients in the community. The role involves scheduled patient visits and case management, working collaboratively with patients, families, caregivers, and the clinical team to support quality care and continuity of services. The nurse reports to the office at 8:00 AM to pick up a company vehicle for scheduled work-related visits. The position is Monday through Friday.
Requirements
- Current, valid Registered Nurse (RN) license.
- Strong communication, organizational, and critical-thinking skills.
- Compassionate, dependable, and patient-focused.
Nice To Haves
- Home health experience.
- One to three years of case management experience.
Responsibilities
- Complete scheduled patient visits.
- Carry out case management responsibilities as scheduled.
- Collaborate with patients, families, caregivers, and the clinical team to support quality care and continuity of services.
