About The Position

Amedisys is hiring a full-time, day-shift RN Case Manager to serve patients in Clearfield County. The role supports hospice patients receiving care at home. The nurse assesses patients, collaborates with the care team on individualized plans of care, provides education to patients and caregivers, and documents patient progress and symptom management. On-call responsibilities are assigned as needed.

Requirements

One year of nursing experience.
Current RN license specific to the state or states where assigned to work.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
Develop, prepare, and maintain accurate and timely individualized patient care progress records according to care center policies.
Record pain and symptom management changes and outcomes as appropriate.
Teach patients, families, and caregivers appropriate patient care techniques.
Communicate regularly with the clinical manager and care team.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Perform other duties as assigned.
