Requirements

• Graduate of an accredited School of Nursing.
• Active, unencumbered Registered Nurse (RN) license in Colorado.
• Current Basic Life Support (BLS) / CPR certification.
• Access to reliable transportation and valid driver's license.
• At least 1 year of home healthcare experience.
• Strong clinical judgment, excellent communication skills, and a passion for 1-on-1 personalized patient care.

Responsibilities

• Comprehensive Nursing Assessments: Evaluate client physical, psychosocial, and environmental status to baseline needs.
• Individualized Care Planning: Develop and update evidence-based care plans tailored to individual client goals and health outcomes.
• Clinical Interventions & Oversight: Deliver targeted nursing interventions, administer medications as needed, and monitor client progress.
• Care Coordination: Collaborate with family members, caregivers, physicians, and interdisciplinary healthcare teams to adjust care plans and initiate specialized referrals.
• Documentation & Safety: Maintain accurate, timely clinical records adhering to safety, infection control, and HIPAA privacy guidelines.
