About The Position
Albany Home Health and Hospice is seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality home health and hospice care. This role focuses on delivering patient-centered care through assessments, care planning, and education in a home-based setting. Signature Healthcare at Home provides professional, home-based medical care for patients needing rehabilitation and expert care following surgery, a hospital stay or other illness. For individuals and their caregivers who are facing chronic health conditions, the experienced team at Signature can help patients achieve their health care goals and promote healing while remaining in the comfort of their home. Signature Healthcare at Home works with each patient’s primary care provider to develop a care plan which will meet their individual needs.
Requirements
- Active RN license (Oregon)
- Graduate of an accredited nursing program
- 2+ years of nursing experience (home health, hospice, or public health preferred)
- Strong clinical, communication, and organizational skills
- Current CPR certification
- Valid driver’s license, reliable transportation, and insurance
Responsibilities
- Perform comprehensive patient and family assessments
- Develop and manage individualized care plans
- Provide skilled nursing care, treatments, and medication administration
- Educate patients, families, and caregivers
- Collaborate with physicians and interdisciplinary care teams
- Maintain accurate and timely documentation
- Supervise home health aides and support staff
- Participate in on-call rotation and quality improvement programs
