About The Position

The RN Case Manager provides support and expertise across the continuum of care, from admission through discharge. The role supports physicians and the interdisciplinary team in facilitating patient care while promoting quality clinical outcomes and patient satisfaction and managing care costs. It integrates and coordinates utilization management, care facilitation, and discharge planning. The RN Case Manager incorporates admission status and medical necessity determinations to help ensure appropriate care and services are delivered efficiently and on time.

Requirements

Bachelor's degree in Nursing required, or three years of acute care hospital experience may be substituted in lieu of the bachelor's degree and experience requirement.
One year of acute care hospital experience required; three years of acute care hospital experience may be substituted in lieu of the bachelor's degree and experience requirement.
Current and valid Registered Nurse (RN) license to work within the state of California required.
Current Basic Life Support for Healthcare Providers certification from the American Heart Association required.
