About The Position
Beyond Hospice is a non-profit hospice provider committed to delivering exceptional end-of-life care with compassion, dignity, and respect. Our interdisciplinary team partners with patients and families to provide personalized care, comfort, and support, ensuring the highest quality of life throughout the hospice journey. We are seeking a Hospice RN Case Manager to plan, coordinate, and provide skilled nursing care to hospice patients and their families. Working collaboratively with an interdisciplinary team, the RN Case Manager plays a vital role in delivering compassionate, patient-centered care focused on comfort, dignity, and quality of life.
Requirements
- Current Minnesota RN license
- Graduate of an accredited nursing program.
- Minimum three (3) years of acute care nursing experience required.
- Valid driver's license, reliable transportation, and current auto insurance.
Nice To Haves
- Hospice or home care experience preferred.
- Strong assessment, communication, and critical thinking skills.
Responsibilities
- Complete comprehensive patient assessments and develop individualized care plans.
- Manage and coordinate care for an assigned hospice caseload.
- Administer medications and treatments as ordered.
- Provide education, guidance, and emotional support to patients and families.
- Collaborate with physicians, social workers, chaplains, aides, and other team members.
- Maintain accurate and timely clinical documentation.
- Participate in interdisciplinary team meetings and care planning.
- Coordinate services, equipment, and resources to support patient needs.
- Participate in on-call and weekend rotation as assigned.
