About The Position
Transitions At Home is seeking to hire a compassionate and experienced Full-Time RN Case Manager to join their growing home health team. This position offers the opportunity to provide personalized care in a home setting, ensuring clients receive the highest quality of service and support throughout their recovery journey. Requirements
- Active Wisconsin Registered Nurse License
- Two (2) years’ nursing experience
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance
Nice To Haves
- 1 year in Home Health
Responsibilities
- Provide skilled nursing care to include assessments, medications, treatments, and coordinate services.
- Counsel the client and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
