About The Position

Westminster Canterbury of Lynchburg, a 5-Star rated Continuing Care Retirement Community in Central VA is seeking a RN Case Manager to join our Hospice team! This is a full-time position with a complete benefits package. The RN Case Manager is responsible for the planning, coordination, and delivery of quality healthcare services, ensuring compliance with corporate standards and regulations. They will implement the plan of treatment, evaluate its effectiveness, and participate in the Interdisciplinary Group (IDG). The role involves ongoing assessment, nursing care, and teaching to clients in various settings, developing and implementing comprehensive care plans, and adhering to professional standards. Maintaining accurate client records, collaborating with other disciplines, and supervising home health/hospice aides and LPNs are key responsibilities. The position also requires staying knowledgeable about regulatory requirements.

Requirements

  • A Valid unencumbered VA RN license is required.
  • Current CPR certification (Basic Life Support).
  • Have a valid Virginia Driver’s License with a good driving record.
  • Commitment to best practices for infection control, including receiving influenza and recommending COVID-19 vaccines.

Nice To Haves

  • Prior Hospice or Home Health Experience is preferred.

Responsibilities

  • Planning, coordination, and delivery of quality health care services.
  • Assuring compliance with corporate standards and federal, state, and local regulations.
  • Implementation of the plan of treatment, executing, organizing, overseeing, integrating, and modifying resources to reach desired outcomes.
  • Evaluating the effectiveness of the plan on an ongoing basis.
  • Participation in the IDG (Interdisciplinary Group).
  • Providing ongoing assessment, nursing care, and teaching to clients in home, assisted living, and long-term care facilities.
  • Assuring accurate evaluation of client needs and developing and implementing a comprehensive care plan.
  • Following current home health and/or hospice nursing professional standards of practice and agency approved protocols.
  • Maintaining current and accurate client records through use of laptop computers and paper/chart documentation.
  • Collaborating with other disciplines and processing internal or external referrals.
  • Supervising and monitoring the performance and skills of Home Health/Hospice Aides and/or Licensed Practical Nurses.
  • Maintaining knowledge of requirements of regulatory agencies, accrediting bodies, and third-party payers.
