About The Position

Westminster Canterbury of Lynchburg, a 5-Star rated Continuing Care Retirement Community in Central VA is seeking a RN Case Manager to join our Hospice team! This is a full-time position with a complete benefits package. The RN Case Manager is responsible for the planning, coordination, and delivery of quality health care services, ensuring compliance with corporate standards and federal, state, and local regulations. They must demonstrate knowledge and support of the philosophy, purpose, goals, and policies of Senior Independence and consistently comply with the Client Bill of Rights. The role involves implementing the plan of treatment by executing, organizing, overseeing, integrating, and modifying resources to achieve desired outcomes, and continuously evaluating the plan's effectiveness. Participation in the IDG is required. Responsibilities include providing ongoing assessment, nursing care, and teaching to clients in home, assisted living, and long-term care facilities. The RN Case Manager must accurately evaluate client needs and develop and implement comprehensive care plans. They are expected to follow current home health and/or hospice nursing professional standards of practice and agency-approved protocols, including disease management programs. Maintaining current and accurate client records using laptop computers and paper documentation is essential, in accordance with policies and guidelines. Collaboration with other disciplines and processing referrals to meet client needs are also key functions. The role includes supervising and monitoring the performance and skills of Home Health/Hospice Aides and/or Licensed Practical Nurses to ensure quality care. Maintaining knowledge of regulatory agency, accrediting body, and third-party payer requirements is necessary.

Requirements

  • A Valid unencumbered VA RN license is required
  • Current CPR certification (Basic Life Support)
  • Have a valid Virginia Driver’s License with a good driving record

Nice To Haves

  • Prior Hospice or Home Health Experience is preferred

  • Assuring an accurate evaluation of client needs and developing and implementing a comprehensive care plan that addresses those needs
  • Following current home health and/or hospice nursing professional standards of practice and agency approved protocols, including disease management programs
  • Maintaining current and accurate client records through use of laptop computers and paper/chart documentation in accordance with corporate policies, federal, state, and local guidelines
  • Collaborating with other disciplines, suggesting and/or processing internal or external referrals in order to meet the comprehensive needs of the clients
  • Supervising and monitoring the performance and skills of Home Health/Hospice Aides, and/or Licensed Practical Nurses, assuring the delivery of quality health care services
  • Maintaining knowledge of requirements of regulatory agencies, accrediting bodies, and third party payers
