About The Position

ComForCare is seeking a dedicated RN Case Manager to provide exceptional patient care in a home setting. This role offers clinical independence and the support of a Great Place To Work® certified company. The position emphasizes work-life balance with autonomous 1-on-1 patient visits and flexible scheduling. The ideal candidate will lead high-quality, patient-centered care, conducting comprehensive nursing assessments, developing individualized care plans, delivering clinical interventions, coordinating care with various stakeholders, and maintaining accurate documentation.

Requirements

Graduate of an accredited School of Nursing.
Active, unencumbered Registered Nurse (RN) license in Colorado.
Current Basic Life Support (BLS) / CPR certification.
Access to reliable transportation and valid driver's license.
At least 1 year of home healthcare experience.
Strong clinical judgment, excellent communication skills, and a passion for 1-on-1 personalized patient care.

Responsibilities

Evaluate client physical, psychosocial, and environmental status to baseline needs.
Develop and update evidence-based care plans tailored to individual client goals and health outcomes.
Deliver targeted nursing interventions, administer medications as needed, and monitor client progress.
Collaborate with family members, caregivers, physicians, and interdisciplinary healthcare teams to adjust care plans and initiate specialized referrals.
Maintain accurate, timely clinical records adhering to safety, infection control, and HIPAA privacy guidelines.
