Part Time RN Case Manager
About The Position
BrightStar Care is seeking a part-time RN Case Manager to join their growing team in the South Minneapolis Area. This role involves working 3 days per week, completing assessments, re-assessments, and some skilled cares for home care private pay and veteran clients. As a non-Medicare agency, OASIS documentation is not required. The nursing team operates as a supportive family, dedicated to helping clients remain safe, happy, and healthy in their homes. The position offers PTO accrual from day one and paid mileage between clients and the office. The primary client base is located in the Minneapolis Metro Area, including Bloomington, Edina, and Eden Prairie.
Requirements
- Active RN license in the state of Minnesota.
- Exceptional interpersonal and communication skills.
- Committed to delivering high-quality, patient-centered care.
Nice To Haves
- Previous experience in a home health or acute care setting is advantageous.
Responsibilities
- Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
- Collaborate with multidisciplinary team members to optimize patient care and outcomes.
- Educate patients and their families on health maintenance and disease prevention.
- Ensure accurate documentation and compliance with all regulatory guidelines.
