Requirements

Registered Nurse (RN) license

Responsibilities

Coordinate patient care from admission through discharge
Assess patient needs and develop individualized discharge plans
Collaborate with providers, nursing staff, patients, families, and community resources
Facilitate transitions of care and connect patients with appropriate post-acute services
Monitor utilization and ensure compliance with regulatory and payer requirements
Promote quality outcomes, patient safety, and efficient resource management
