About The Position

The Project is actively seeking an HIV Case Manager to oversee a caseload of individuals living with HIV in the Illinois Quad Cities and surrounding areas. The Case Manager will collaborate with community organizations and assess clients' biopsychosocial needs. The Project has been providing affirming healthcare and supportive services to people living with HIV, those most at risk, and the broader LGBTQ+ community for 40 years.

Requirements

  • 6 years of relevant work experience with demonstrated knowledge of the field. No degree required.
  • Associate's degree in social work, human services, social science, or health education, plus 4 years of case management or related experience.
  • Bachelor's degree in social work, human services, social science, or health education from an accredited college or university.
  • Strong cultural awareness of people living with HIV/AIDS, men who have sex with men, communities of color, and people with substance use issues, and how this may affect or influence service delivery.
  • Desire and ability to engage in equity and inclusion initiatives and discussions in the workplace, including but not limited to engaging in training workshops, team and department meetings, and critical conversations for how The Project can better meet the needs of those we serve.
  • Valid Driver’s License, current vehicle insurance coverage, and reliable access to a working vehicle.

Responsibilities

  • Managing a caseload of 45-50 clients living with HIV and ensuring eligibility is maintained for Ryan White Part B services which includes updating income, residency, insurance, and lab work on a bi-annual basis.
  • Assisting clients with overcoming identified barriers to medication and treatment adherence including, but not limited to, housing instability, inadequate nutrition, transportation barriers, substance use disorders, and mental health needs.
  • Maintaining close advocate relationships with clients to include face-to-face contact, at minimum, on an annual basis.
  • Developing meaningful rapport and advocating for clients served by maintaining direct communication with community-based organizations and medical providers.
