RN Case Manager (Mon-Fri Day Shift)
About The Position
DDN Consulting Services is hiring a compassionate Registered Nurse to case manage adults with developmental and intellectual disabilities and support the staff who care for them in group homes. The role offers a consistent caseload and an opportunity to advocate for individuals’ needs as part of a nursing team focused on career advancement and longevity. The position follows a Monday-through-Friday, 8 a.m. to 5 p.m. schedule, with no nights, weekends, holidays, or on-call work. It involves travel to select group homes in Waterbury, Watertown, Thomaston, Naugatuck, Bristol, and surrounding towns. DDN Consulting Services provides nursing care and support for adults with developmental disabilities.
Requirements
- Active Connecticut RN license.
- Valid driver's license and reliable transportation.
Nice To Haves
- One to two years of nursing experience preferred.
- Experience with developmental disabilities is a plus.
Responsibilities
- Provide oversight, training, and documentation.
- Develop plans to advocate for individuals’ health care needs.
- Write nursing reviews.
- Monitor medical and dental records.
- Review documentation to ensure certified staff follow procedures for reporting medication-administration errors.
- Supervise staff during their initial medication-administration worksite experience.
- Conduct onsite training.
- Case manage individuals with developmental and intellectual disabilities and provide vital information to group-home staff.
- Travel to select group homes in Waterbury, Watertown, Thomaston, Naugatuck, Bristol, and surrounding towns.
