About The Position

Amedisys is hiring a full-time registered nurse case manager to provide home health care. The role focuses on assessing patients, collaborating with the care team on plans of care, supporting patients’ health and independence through teaching and rehabilitation, and coordinating services based on patient needs. The case manager supervises LPNs and HHAs, communicates patient progress, completes documentation, provides staff education, and performs assigned on-call responsibilities. The posted salary range is $75,000–$90,000.

Requirements

• At least one year of clinical experience as a registered nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement plans of care.
• Make referrals to other disciplines when indicated by the patient's needs, or document the rationale for not making a referral.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
