About The Position

The individual in this position is responsible for coordinating patient care activities to conform to evidence-based practice and/or regulatory/payor requirements. This position integrates functions typically considered to be case management, care coordination and/or utilization management, and discharge planning assessment.

Requirements

Current CA RN license and maintain current
Minimum five (5) years of clinical experience and 1 year of previous Case Management experience or a UCI employee who meets the clinical requirements and has a Bachelor's degree in Nursing
External candidates must have a Bachelor's degree in Nursing or above
Must possess the skills, knowledge and ability essential to the successful performance of assigned duties
Must possess complete understanding of acuity levels for specific patient population
Must demonstrate customer service skills appropriate to the job
Familiarity of Joint Commission, CMS, CDPH and CCS requirements
Excellent written and verbal communication skills in English
Ability to manage multiple concurrent projects and maintain a work pace appropriate to the workload
Ability to facilitate and lead interdisciplinary rounds
Ability to establish and maintain effective working relationships across the Health System
Position involves standing and walking for long periods of time, approximately 75% of the workday.
Must have the dexterity, coordination, and visual and auditory acuity to perform all job responsibilities.
Must be able to communicate effectively, both verbally and in writing.
Must be free from signs of illness and infection.
Must be able to provide proof of work authorization
