About The Position

This role involves integrating health care for clients from pre-admission to post-discharge through the coordination and sequencing of client care. This integration enhances patient/resident flow and interdisciplinary communication, promoting early intervention. The position seeks to evaluate outcomes based on integration among established clinical, financial, and utilization data. Functions in structured and unstructured health care settings, utilizing independent nursing judgment. Knowledge of and utilization of appropriate age-specific structured care methodologies are required. The role involves working with clients in all age groups and requires the ability to communicate effectively with patients, families, and the healthcare team. The RN Case Manager will coordinate, facilitate, and negotiate with others, monitor and evaluate patient/resident responses, and be fiscally responsible. Independence, assertiveness, critical thinking, clear documentation, problem-solving skills, self-motivation, and the ability to work with little direction are essential. The role also requires incorporating teaching/learning principles, facilitating groups, implementing projects, establishing priorities, and possessing strong flexibility and organizational skills. Knowledge of reimbursement for services provided is necessary. The position may involve exposure to communicable diseases, hazardous materials, and injury. It is subject to multiple sensory and environmental stressors.

Requirements

  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Currently holds an unencundered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific competencies and certifications.

Nice To Haves

  • Bachelor's degree in Nursing strongly preferred.
  • Minimum of two years’ clinical experience preferred.
  • Prior home health experience strongly preferred.
  • OASIS experience strongly preferred.
  • Depending on location, a valid drivers license may be required.

Responsibilities

  • Integrates health care for clients from pre-admission to post discharge.
  • Coordinates and sequences the client’s care.
  • Evaluates outcomes based on an integration among established clinical, financial and utilization data.
  • Utilizes independent nursing judgment when integrating health care.
  • Utilizes appropriate age-specific structured care methodologies, such as protocols/integrated clinical pathways/guidelines/standards of care relating to the overall health care needs of neonatal, pediatric, adolescent, adult and/or geriatric patients/residents.
  • Works with growth and development needs of client populations in clinical area.
  • Communicates with patients/residents, family members and others on the health care team.
  • Coordinates, facilitates, and negotiates with others.
  • Monitors, evaluates and trends patient/resident responses utilizing structured care methodologies.
  • Controls variances and duplication.
  • Demonstrates independence, assertiveness and critical thinking when working with patients/residents and co-workers.
  • Documents clearly.
  • Solves problems.
  • Incorporates teaching/learning principles and adapts teaching to age group.
  • Facilitates groups and implements projects when necessary.
  • Establishes priorities, has strong flexibility and organizational skills.
  • Is knowledgeable about reimbursement for services provided.
