Functions in structured and unstructured health care settings, utilizing independent nursing judgment. Knowledge of and utilization of appropriate age-specific structured care methodologies are required. The role involves working with clients in all age groups and requires the ability to communicate effectively with patients, families, and the healthcare team. The RN Case Manager will coordinate, facilitate, and negotiate with others, monitor and evaluate patient/resident responses, and be fiscally responsible. Independence, assertiveness, critical thinking, clear documentation, problem-solving skills, self-motivation, and the ability to work with little direction are essential. The role also requires incorporating teaching/learning principles, facilitating groups, implementing projects, establishing priorities, and possessing strong flexibility and organizational skills. Knowledge of reimbursement for services provided is necessary. The position may involve exposure to communicable diseases, hazardous materials, and injury. It is subject to multiple sensory and environmental stressors.
