About The Position
Grace Unlimited Home Health Care is seeking experienced PRN RN Case Managers to provide high-quality home healthcare to clients primarily in Dallas County, with coverage in surrounding Dallas–Fort Worth areas as needed. This role is ideal for nurses seeking flexible, PRN work while delivering compassionate, patient-centered care in the home. You’ll have the freedom to focus on personalized care while maintaining control over your schedule.
Requirements
- Graduate of an accredited nursing program
- Active Texas RN License
- Valid Driver’s License and reliable transportation
- Minimum 1 year of home health experience
- Ability to travel locally within Dallas County
Nice To Haves
- Experience with OASIS/Kinnser preferred
- Flexible availability for PRN visits
Responsibilities
- Provide professional, compassionate nursing care in clients’ homes
- Perform assessments and manage individualized care plans
- Accurately document care using OASIS/Kinnser
- Manage diverse patient needs in a home health setting
- Respond to PRN and urgent visit needs as assigned
