RN Case Manager
About The Position
The RN Case Manager, also referred to as a Registered Nurse Case Manager or RN Clinical Case Manager, oversees and coordinates patient care from admission through discharge to support quality, cost-effective outcomes. The role assesses patients’ needs, develops care plans, and works with physicians, nurses, social workers, and other providers to coordinate care transitions. Responsibilities also include discharge planning, utilization review, and helping patients navigate insurance and healthcare systems. This is a full-time day-shift position at Riverview Regional Medical Center in Gadsden, Alabama. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with 8-hour shifts.
Requirements
- Current Alabama state RN license.
- BSW or another bachelor’s degree in a related field.
- Current CCM certification or obtain it within one year of hire.
- Basic to intermediate computer skills and knowledge.
- Complete Behavioral Violence Prevention Training within three months of hire and maintain it current.
Nice To Haves
- Five years of acute care experience.
- At least one year of experience in case management, discharge planning, or management.
- Current BLS (AHA) certificate.
- Knowledge of Milliman Criteria and InterQual Criteria.
Responsibilities
- Evaluate patients’ medical, psychosocial, and discharge needs upon admission and throughout their stay.
- Develop and implement individualized care plans in collaboration with the healthcare team to ensure effective treatment and timely interventions.
- Coordinate safe and appropriate discharge plans, including referrals to rehabilitation, home health, or long-term care facilities.
- Monitor the use of hospital resources and services to ensure appropriate care and avoid unnecessary treatments or extended stays.
- Help patients and families understand care plans, post-discharge instructions, and available resources.
- Advocate for patient needs and preferences, ensuring care is aligned with their goals and values.
