About The Position

Sanford Health, the largest rural health system in the United States, is dedicated to transforming the health care experience and providing access to world-class health care in America’s heartland. Good Samaritan Society – Preston provides home care services for southeast Minnesota, including the counties of Freeborn, Mower, Fillmore, Houston, southern Olmsted, and southern Winona. This full-time position, requiring 64 hours per bi-weekly pay period, primarily supports the Austin, MN area with day shifts and a flexible schedule, including an on-call weekend rotation. The RN Case Manager integrates health care for clients from pre-admission to post-discharge through the coordination and sequencing of care. This role enhances patient/resident flow and interdisciplinary communication to promote early intervention. The position involves evaluating outcomes based on established clinical, financial, and utilization data, functioning in structured and unstructured health care settings, and utilizing independent nursing judgment. The Case Manager will work with various age groups, including neonatal, pediatric, adolescent, adult, and geriatric patients/residents, and must be able to communicate effectively with patients, family members, and the healthcare team. The role requires fiscal responsibility, problem-solving skills, self-motivation, and the ability to incorporate teaching/learning principles.

Requirements

  • Must be licensed RN in Minnesota
  • Must have a current driver’s license
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state
  • Obtains and subsequently maintains required department specific competencies and certifications
  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA)
  • Demonstrate independence, assertiveness and critical thinking when working with patients/residents and co-workers

  • Works with little direction
  • Ability to incorporate teaching/learning principles and adapt teaching to age group
