About The Position

Guthrie Home Care New York is hiring a Full‑Time Registered Nurse (RN) Case Manager to join their team based out of Vestal, NY. This role involves supporting patients throughout the community with high‑quality, compassionate home health care. RN Case Managers are critical thinkers and skilled clinicians who play a vital role in helping patients safely recover at home after hospitalization, illness, or injury. The position is responsible for the case management and provision of quality home nursing services to patients within the agency’s philosophy, policy, and Standards of Community Health Nursing Practice, offering meaningful work, autonomy, and a supportive team.

Requirements

  • Graduation from an approved School of Nursing
  • One‐year previous nursing experience with Medical/Surgical Nursing required
  • Must have reliable transportation
  • Must have excellent verbal and nonverbal communication skills
  • Independent judgement and excellent planning and problem-solving skills are required
  • Must be able to accept on call assignments

Nice To Haves

  • Bachelor’s Degree in Nursing preferred
  • Prior Public Health/Community Health experience preferred

Responsibilities

  • Plans, implements, manages, and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient’s needs are met and quality care provided in accordance with Federal, State, and program guidelines
  • Is responsible for ensuring that he or she can meet the yearly competency requirements
  • Attends staff meetings and educational opportunities
  • Must provide nursing care in the patient’s environment
  • Manages the activities of the multi‐disciplinary team providing care to patient
  • Demonstrates effective time management, flexibility and priority setting in patient care
  • Must have excellent verbal and nonverbal communication skills and facilitate communication between team members and patients
  • Independent judgement and excellent planning and problem‐solving skills are required
  • Is responsible for LPN supervision
  • Must be able to accept on call assignments
  • Travel for this position is always required
