About The Position
National Church Residences Home Health is a leading Home Health & Hospice agency known for delivering compassionate, high-quality care. As part of National Church Residences —the nation’s largest not-for-profit provider of affordable senior housing and services—we are committed to advancing better living and care for seniors. We are currently seeking a dedicated and compassionate Registered Nurse Case Manager to join our Home Health team. The ideal candidate will embrace our mission and provide skilled nursing care while coordinating the overall care plan for hospice patients and their families.
Requirements
- Graduate of an accredited nursing program with a current RN license.
- Minimum of two years of RN experience required.
Nice To Haves
- Previous hospice or home health experience is strongly preferred.
