About The Position

The RN Case Manager is responsible for coordinating and managing patient care across primary care and specialty services. This role focuses on improving patient outcomes through care coordination, referral management, patient education, and collaboration with multidisciplinary healthcare teams.

Requirements

  • Graduate of an accredited School of Nursing
  • High school diploma or equivalent required
  • Minimum 1 year of general nursing experience, OR 2 years of general nursing experience including ambulatory care or case management
  • Active, unrestricted RN license (or ability to obtain prior to start)
  • Basic Life Support (BLS) certification
  • Clinical competency within 3 months of hire (renewed every 3 years)
  • Strong case management and care coordination skills
  • Ability to manage multiple patients and priorities effectively
  • Excellent communication and interpersonal skills
  • Strong critical thinking and problem-solving abilities
  • Experience in fast-paced clinical environments
  • Proficiency in documentation and EHR systems

Nice To Haves

  • Bachelor’s Degree preferred
  • Case Management Certification (preferred)

Responsibilities

  • Care Coordination & Case Management
  • Manage an active patient caseload across multiple services
  • Develop individualized care plans in collaboration with providers and care teams
  • Coordinate multidisciplinary care conferences for high-risk patients
  • Track referrals, diagnostics, and treatment progress
  • Ensure timely follow-up and resolution of care gaps or delays
  • Clinical Support
  • Provide nursing care including assessment, documentation, medication administration, and patient education
  • Participate in triage and emergency response as needed
  • Support outpatient clinic workflows and specialty services
  • Specialty & Program Support
  • Manage specialty referrals and scheduling coordination
  • Support case management for specialized programs (e.g., intake, tracking, education, follow-up)
  • Assist with coordination of telehealth visits and external provider consultations
  • Coordinate diagnostic testing and follow-up care
  • Patient Advocacy & Communication
  • Serve as a primary patient advocate in care coordination
  • Educate patients and families regarding treatment plans and follow-up care
  • Facilitate communication between providers, patients, and external facilities
  • Quality & Compliance
  • Maintain accurate and timely documentation in the electronic health record (EHR)
  • Adhere to infection control and patient safety standards
  • Participate in competency training and continuous improvement initiatives
