About The Position

This hybrid role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), other assessments and services, occasional Skilled Nursing visits, as well as new hire orientations, trainings, in-services, and supervisory reviews. The primary service area is centralized in Highland County, with typical travel distances in a 10-60 minute radius, with occasional travel up to 90 minutes also required. Option for travel up to or beyond a 2-hour radius is available if additional hours are desired (PT or FT), which may include Clermont, Hamilton, Clinton, Brown, Adams, and other counties. Highland County is central to all clients in this service area, but qualified applicants residing outside of but near Brown County will be considered so long as travel requirements are fulfilled. This is a Part-time or PRN position, both for long-term employment opportunity that can be built to fit your schedule. Typical travel requires a 30-60 minute daily radius (or less), with travel between 1-2 hours (max.) required on occasion. If PRN schedule is preferred, please note that hours and shifts vary week to week, including some weeks having low or no hours at all. A Full-time option is available if desired, if willing to travel up to or beyond 2+ hours as needed. Candidates seeking long-term employment are preferred, however short-term or temporary employment will be considered. For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Business Administrator, QA / Compliance leadership, and other positions will be supported.

Requirements

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • 1 year Experience in case management and home health required, with 2+ years of home health experience highly preferred.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.

Nice To Haves

  • 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.

Responsibilities

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
