About The Position

This position will be hired as PRN or Part-Time only. Additional Part-Time hours are also available with a 2-hour travel radius to "Secondary" locations. Full-Time may be available in the future but not at this time and is not guaranteed. This is a regional leadership role within our agency, requiring travel as needed throughout Dayton and Metro-Dayton areas where clients and caregivers reside. This role works closely with and reports directly to our SW OH Regional Program Administrator (local) and/or Director of Clinical Administration & Operations (corporate). For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported. Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. Requires back-up and occasional Skilled Nursing visits and services including med sets, dressing changes, etc. At HCN, this is a field staff leadership role and the right RNCM has a passion for delivering consistently excellent care and care experiences to clients, and the ability to do so directly and through the thoughtful leadership of others. This position requires travel to multiple counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel).

Requirements

  • At least 1 year of experience is required in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent).
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • Minimum of 1 year of experience in home health agency or equivalent setting.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience with OASIS and NON-OASIS assessments is also required.
  • Strong background in nursing, patient care, and confidentiality.
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid).
  • Familiarity with HIPAA regulations and medical terminology.
  • Willingness and ability to perform required physical demands.
  • Consent + Ability to pass a State & Federal background check (including fingerprints).
  • 18yo + active Driver's License / State ID + US Work Authorization.
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients (up to at least 2 hours is required).

Responsibilities

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Coordinate, re-evaluate and confirm the medical necessity of continued home health care services for patients.
  • Provide patient care and ensure optimal outcomes.
  • Oversee patient services and provide expert nursing care.
  • Submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • Ensure successful communication with and between physicians, agency staff, service administrators, other caregivers and clinicians, and the client and their family.
