About The Position

The RN Case Manager position at Sanford Health provides expertise and leadership in effective resource management for patient care delivery across the care continuum for assigned patient populations. This role involves collaborating with patients, families, licensed practitioners, and other inter-professional team members to ensure care coordination, transitional care planning, and facilitation of discharge needs. The primary goal is to enhance the quality of clinical outcomes and patient satisfaction while managing care costs. The Case Manager promotes patient wellness through evidence-based practice, improved care outcomes, efficient utilization of health services, and ensuring appropriate levels of care during hospitalization and post-discharge. Key responsibilities include providing discharge-planning education, making necessary referrals to inter-professional teams, and initiating links with post-acute external care resources. The role also requires anticipating and identifying patient educational needs and discharge planning issues to expedite early discharge. Functioning as a patient advocate across various care settings, the RN Case Manager utilizes the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to deliver individualized nursing care. They collaborate with physicians and other colleagues to plan, implement, and evaluate care, demonstrating competency within the full scope of nursing expertise and adhering to age and population-specific standards. The position encompasses caring for patients through all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up, and involves planning and coordination of care, patient assessment, patient, caregiver, and family education, and triage, all while functioning within the scope and standards of nursing practice as outlined by the Nurse Practice Act and the American Nurses Association.

Requirements

  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtain and subsequently maintain required department specific competencies and certifications including Basic Life Support (BLS) within 60 days of hire.
  • Bachelor's degree in Nursing (required in Fargo, Sioux Falls, Bismarck, Fargo Network, and Sioux Falls Network; strongly preferred in Bemidji, Wisconsin, Michigan, and Rapid City. For Bismarck, enrollment in a Bachelor's degree program within one year of hire with completion within three years is acceptable. For Fargo Network and Sioux Falls Network, employees hired after January 2025 without a Bachelor's degree may be considered with an approved educational plan for completion within five years, requiring Nursing Executive approval to waive the initial requirement).

Nice To Haves

  • Minimum of two years’ clinical experience preferred.
  • Certification in Case Management (CCM), or a certification in their patient population specialty, is encouraged when eligible.

Responsibilities

  • Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients.
  • Collaborate with other inter-professional colleagues, including physicians, to plan, implement and evaluate care.
  • Demonstrate competency and practice within the full scope of nursing expertise/knowledge and utilize appropriate age and population specific standards as designated in their assigned clinical setting.
  • Care for patients in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up as patients move along the continuum of care.
  • Responsible for the planning and coordination of care, patient assessment, patient, caregivers, and family education, triage, and various other nursing interventions.
  • Function within the scope and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure.
