The role integrates utilization management, care coordination, and transition planning functions. The Inpatient Case Manager (ICM) has overall accountability for a designated caseload and plans effectively to meet member’s needs, manage the length of stay, and promote efficient utilization of resources. Based on an initial review of member care needs at the time of admission, the ICM establishes a discharge/transition plan in collaboration with the hospitalist team. On an ongoing basis, the ICM reviews and identifies changes in member care needs, adjusting the discharge/transition plan accordingly to ensure the transition to a lower level of care is effectively executed to prevent adverse outcomes and possible readmissions. As a member of the interdisciplinary team, the ICM with UM experience supports the hospitalist in facilitating inpatient member care with the objective of enhancing the quality of outcomes and satisfaction while managing the cost of care.
