About The Position

The Registered Nurse (RN), Hospice provides end-of-life care to patients with a terminal diagnosis needing hospice services in their home or community setting.

Requirements

  • Graduate of an accredited school of nursing required.
  • Must possess and maintain State of Minnesota RN licensure.
  • Possess a valid driver’s license with good driving record, and proof of valid insurance.
  • Must have reliable transportation.
  • CPR Certification required.
  • Excellent interpersonal, collaboration, and relationship building skills to effectively work with a diverse group/variety of people and personalities, including developing relationships with vendors, family members, volunteers, clients and staff.
  • Able to provide client care planning and client teaching skills.
  • Able to plan, prioritize, coordinate, and manage own work.
  • Able to work unsupervised, make decisions and solve routine problems independently, effectively and creatively.
  • Work with a proactive, ownership approach to work and operate with a sense of urgency.
  • Understand and maintain confidential nature of organization and client information, including and in accordance with HIPAA regulations.
  • Self-motivated, fast learner shows initiative, willing to seek out client and staff needs and meet them.
  • Strong computer skills, including Microsoft Word, Excel, Outlook and other clinical applications.
  • Must be able to successfully pass training and education modules, and exhibit competency with medication administration and documentation.

Nice To Haves

  • One year of Clinical Nursing experience preferred.
  • Med-Surg, TCU, LTC, Assisted Living, Home Health, or Hospice experience preferred.

Responsibilities

  • Provide end-of-life care to patients with a terminal diagnosis needing hospice services in their home or community setting.
  • Assist residents with mobility needs such as transfers, ambulation, escorts or use of assistive devices.
  • Follow established safety protocol and use proper body mechanics.
  • Work as an effective and proactive team-player; understands the importance of supporting the organization, clients, customers and other Sholom employees.
  • Display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization in accordance with our PROUD standards.
  • Communicate clearly and effectively both orally and in writing to all levels of people inside and outside the organization, including possessing good English-speaking skills, fluency and understandability.
  • Develop and give oral presentations to large and small groups of individuals inside and outside the organization.
  • Follow Sholom safety guidelines in all duties and responsibilities.
  • Perform the duties of unlicensed staff.
  • Follow and adhere to Universal Precautions and Infection Control procedures.
