About The Position

We are seeking a highly skilled RN Case Manager to provide end-of-life care to patients in hospice care. The role works closely with physicians, nurses, and other healthcare professionals to provide high-quality care, manage patient care according to individual needs, and coordinate care with other providers. The RN Case Manager ensures patients receive the support and resources needed to manage symptoms and improve quality of life, while maintaining accurate and up-to-date patient records.

Requirements

• Active RN license in the state of California.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong organizational and time management skills.
