About The Position

Agrace is seeking a skilled RN who values meaningful, patient-centered care to make a difference as a Nurse Case Manager. This role involves providing nursing care and case management through assessments, updating the plan of care, interventions, communication, and documentation, all while upholding Agrace's philosophy, goals, standards, regulations, and policies.

Requirements

  • ADN or BSN required
  • Current, unrestricted Registered Nursing License in state of employment
  • One-year nursing experience, preferred
  • Valid driver’s license, auto insurance, and access to a reliable vehicle
  • Must be at least 18 years of age
  • Basic Microsoft Office Skills
  • Intermediate electronic health records skills
  • Ability to use proper body mechanics, safe patient handling, and standard precautions

Responsibilities

  • Conduct patient and family assessments, including physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs.
  • Collect data from patients, families, and the interdisciplinary team.
  • Identify, interpret, and document assessment data.
  • Document and report significant changes in condition or unusual incidents.
  • Collaborate with the Interdisciplinary Team (IDT) and primary care provider to determine hospice eligibility.
  • Respond to patient and family needs or changes in condition.
  • Provide interventions, treatments, and pain management.
  • Evaluate the effectiveness of interventions and adjust as needed.
  • Provide patient and family education on pain, symptom management, and end-of-life care.
  • Formulate, implement, and revise individualized Plans of Care (POC) in collaboration with the IDT, patient, and family.
  • Develop outcome measures and establish care priorities.
  • Document interventions, outcomes, and changes in patient condition in the EHR.
  • Delegate visits to appropriate disciplines and manage visit sets.
  • Oversee the work of Certified Nursing Assistants (CNA) and Licensed Practical Nurses (LPN).
  • Participate in meetings to plan and evaluate patient/family services.
  • Precept new staff and students.
