About The Position

Patriot At Home is a veteran-owned and family-operated company seeking a dedicated RN Homecare Field Case Manager. This position is paid at a competitive salary with the opportunity to earn extra money by picking up visits at our per diem rate. Mileage reimbursement is provided for company travel. Medical, Dental, Vision, and Supplemental insurance begin immediately upon hire. We also offer a 401K with 4% company matching.

Requirements

Graduate from an accredited School of Nursing
Licensed in the state as a Registered Nurse
Knowledge of Medicare and Medicaid guidelines
Criminal background check
Must have a current CPR (BLS) certification
Valid driver's license required
Ability to access patients' homes, which may not be wheelchair accessible.
Sufficient hearing, eyesight, and physical dexterity for patient assessments and care
Standard nursing activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients

Nice To Haves

One or more years of experience in community/home health agency or hospital preferred

Responsibilities

Have coordination of total patient care
Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis
Perform admission, transfer, recertification, resumption of care, and discharge Oasis for the home care patient
All other duties as assigned
