About The Position

RN Case Managers at VNH screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient. The RN Case Manager develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission. They periodically review the patient care plan and recertify the patient care plan as appropriate. They coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate, and advise other disciplines of discharges in a timely manner. They provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals, and communicate significant changes to the patient's physician and VNA colleagues. They document evaluations, treatment goals, and objectives. They teach the patient and caregiver self-care techniques as appropriate. They provide medication, diet, and other instructions as ordered by the physician and utilize opportunities for health counseling with patients and caregivers.

Requirements

• Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license.
• Graduate of an accredited school of nursing.
• 2 years of experience in Home Health
