RN Case Manager, Home Health (Weekend Warrior) Full Time West Valley
About The Position
Assisted is experiencing tremendous growth, offering stability and opportunity for advancement. We are an award-winning home health team looking for an RN to cover the East Side of Arizona on a Friday, Saturday, Sunday, and Monday schedule. This is a full-time position covering the entire West Valley area. Experience with Oasis paperwork is required. The role includes on-call responsibilities for the entire day/night and requires conducting 4 Starts of Care (Oasis SOC) per day. Compensation includes $110.00 per RN SOC visit and $85.00 per ROC/Recert/Eval visit. A daily compensation of $500 per workday is provided, which includes all work time, on-call time, routine and other visits (except SOC), timely submission bonuses, and phone/mileage reimbursement.
Requirements
- Graduate of an approved school of professional nursing.
- Current, active license in the state of Arizona Board of Nursing, pursuant to ARS Title 32, Chapter 15 including fingerprint checks.
- Current CPR, health clearance, TB clearance.
- Must have experience with Oasis Paperwork.
- Must be available for on-call (for complete day/night).
- Must conduct 4 Starts of Care (Oasis SOC) per day.
Responsibilities
- Complete initial and on-going comprehensive assessments of the patient’s needs
- Provide service and /or treatments requiring substantial and specialized nursing skills
- Initiate appropriate preventative and rehabilitative nursing procedures
- Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs
