About The Position

The Home Health and Hospice RN Case Manager assesses, plans, implements, and evaluates patient care in order to achieve optimum patient outcomes for patients as appropriate to service area. Age specific admission and ongoing assessments are completed, documented and used to develop a plan of care. nursing care and provision of individualized care for patients. Utilizes delegation and critical thinking skills to provide quality service through a team oriented, collaborative approach to patient care. This full-time position is with Home Health. The Home Health RN Case Manager Interacts with patients, families, visitors, and all members of the health care team to develop, coordinate, and manage the plan of care. Manages and oversees delivery of patient care such as and not limited too: Maintains patient, staff and customer confidentiality. Demonstrates knowledge of the principles of life span growth and development and assesses data reflective of the patient's status. Interprets the appropriate information needed to identify each patient's requirements relative to his or her age specific needs and provides care according to departmental policies and procedures. Work involves application of advanced knowledge and skills of patient care. Duties are varied and complex. Accountable to resolve issues as determined by license. Handles routine issues, follows up to conclusion on matters, while referring only the most complex or higher level issues and problems to the Manager, Team Lead, or Supervisor. Must be able to work in a fast paced environment with the ability to effectively prioritize and demonstrate multi-tasking skills. Physical requirements include: lifting, prolonged periods of standing, bending, pushing, pulling, reaching, walking, etc. May be asked to provide care at other Carilion facilities to meet patient needs. Some positions may be required to take call. May be required to travel in adverse weather. Care provided in a variety of home settings. Requirements

  • Graduate of accredited Registered Nurse Program.
  • Currently licensed as Registered Nurse in the state of Virginia or approved compact state.
  • Must have valid driver’s license, automobile insurance, and personal vehicle for making home visits.
  • Keyboarding skills required for automated computer systems, including electronic medical records.
  • Demonstrates knowledge of the principles of life span growth and development and assesses data reflective of the patient's status.
  • Interprets the appropriate information needed to identify each patient’s requirements relative to his or her age specific needs and provides care according to departmental policies and procedures.
  • Work involves application of advanced knowledge and skills of patient care.
