About The Position

This role integrates health care for clients from pre-admission to post discharge through the coordination and sequencing of the client’s care. This integration enhances patient/resident flow and interdisciplinary communication, promoting early intervention. The position seeks to evaluate outcomes based on an integration among established clinical, financial, and utilization data. The role functions in structured and unstructured health care settings, utilizing independent nursing judgment when integrating health care. Knowledge of and utilization of appropriate age-specific structured care methodologies, such as protocols/integrated clinical pathways/guidelines/standards of care relating to the overall health care needs of neonatal, pediatric, adolescent, adult, and/or geriatric patients/residents is required. The ability to work with growth and development needs of client populations in the clinical area, and potentially with clients in all age groups, is necessary. Strong communication skills with patients/residents, family members, and the health care team are essential. The role involves coordinating, facilitating, and negotiating with others, monitoring, evaluating, and trending patient/resident responses utilizing structured care methodologies. Fiscal responsibility, controlling variances and duplication, and demonstrating independence, assertiveness, and critical thinking are key. The ability to document clearly, solve problems, and work with little direction is required. The role also involves incorporating teaching/learning principles and adapting teaching to age groups, facilitating groups, and implementing projects when necessary. Establishing priorities, strong flexibility, and organizational skills are a must. Knowledge about reimbursement for services provided is required. The role may involve exposure to communicable or infectious disease, hazardous materials, and injury from performance of assigned duties. It is subject to multiple sensory and environmental stressors.

Requirements

  • Graduate from an accredited nursing program, including, but not limited to, those accredited by the Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Currently holds an unencunbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Obtains and subsequently maintains required department specific competencies and certifications.
  • Demonstrates independence, assertiveness and critical thinking.
  • Ability to document clearly.
  • Ability to solve problems.
  • Self motivated.
  • Works with little direction.
  • Ability to incorporate teaching/learning principles and adapt teaching to age group.
  • Ability to facilitate groups and implement projects when necessary.
  • Must be able to establish priorities, have strong flexibility and organizational skills.
  • Must be knowledgeable about reimbursement for services provided.

Nice To Haves

  • Bachelor's degree in Nursing strongly preferred.
  • Minimum of two years’ clinical experience preferred.
  • Depending on location, a valid drivers license may be required.
